Consent Agreement for Low Voltage Cabling Group Partnership

This Consent Agreement ("Agreement") is entered into between the vendor ("Vendor") Dyson IT Works LLC (DITW) and Low Voltage Cabling Group, INC ("LVCG") to establish the terms and conditions for becoming a Partner. By checking the box below, Vendor agrees to comply with the following terms and conditions:

I agree to the Net30 payment terms and conditions, which require invoices to be paid within 30 days of the invoice date.

I agree to upload all invoices for completed projects to the designated platform no later than 7 days after project completion. Failure to submit invoices within the specified timeframe may result in delayed payment.

I agree to complete and return all required onboarding documentation, including Vendor's Certificate of Insurance (COI) before payment can be processed.

I agree to upload all deliverables to the Dyson IT Works vendor portal uploads and consent page. This includes all necessary project-related files, documentation, and other deliverables.

I agree to complete IRS Form W-9 Form W-9 (Rev. October 2018) (irs.gov) – complete, download, sign, and submit along with your onboarding package.

By checking the box, Vendor confirms that they have read, understood, and agree to comply with all the terms and conditions outlined in this Consent Agreement.
